Data Export Notice for Isuzu Connect

This Data Export Notice (the “Notice”) between, on the one hand, you and the company you represent (“Dealer”) and, on the other hand, Decisiv, Inc. (“Decisiv”), governs the relationship between Dealer, Decisiv and the provider of Dealer’s dealer business systems (“your DBS”) that you have identified to Decisiv as part of the registration process, and is effective on your acceptance of this Notice by clicking the “Accept” button. You represent and warrant that you have the right and ability to act on behalf of Dealer and to bind Dealer with respect to the consents, rights and obligations set forth in this Notice.

BY CLICKING THE ACCEPT BUTTON, YOU ACKNOWLEDGE THAT YOU HAVE READ ALLOF THE TERMS AND CONDITIONS OF THE NOTICE, UNDERSTAND THEM, AND AGREE TO BE BOUND BY THEM.

Decisiv and Isuzu Commercial Truck of America, Inc. (“Isuzu”) have entered into a distribution and services agreement pursuant to which Isuzu makes available the Decisiv Service Relationship Management platform to its dealers and customers under the brand name “Isuzu Connect”. Dealer is executing this Notice in connection with the process under which it will acquire access to Isuzu Connect. Isuzu Connect requires a baseline level of integration with your DBS. To facilitate the implementation of Isuzu Connect for Dealer and the integration of Isuzu Connect with your DBS, Dealer hereby authorizes Decisiv to perform an initial import of Dealer’s customers, assets, and operations data from your DBS. Dealer hereby authorizes your DBS to provide this export of customers, assets, and operations data from your DBS to Decisiv to be imported to Isuzu Connect. Dealer further agrees that Decisiv may share this executed Notice with your DBS as proof of such authorization.
